If you have a dental emergency after hours you can reach us at our office number (925-672-7997). (If you need to change an appointment or have a non-emergency concern, please call during normal business hours.)
For your convenience, we have provided our new patient forms in the link below for you to fill out prior to your arrival. Or you can complete your paperwork at our practice before your appointment. We’ve also provided
our privacy practices for your review.
Make sure to complete both forms: New Patient Registration and Medical History Form. After this initial registration we will ask you for updates every 2 years.
For Your Review:
We do accept most PPO dental plans and are considered in network for Delta Dental.
If you don’t currently have insurance coverage, we offer options:
*If you are 62 years or older, we offer a 10 percent courtesy off all our treatments.
