At the conclusion of your exam, a treatment plan will be provided to you in writing for all recommended procedures displaying our fees and the courtesy applied.
Prices per calendar year:
Coverage is in effect for one calendar year, ending December 31st. Any prepaid service that is unused by December 31st will NOT be rolled over into the next year.
Coverage begins when membership has been paid in full. Members will be notified one month prior to membership renewal.
Any treatment must be paid in full at time of service unless pre-approved financial arrangements have been made. Interest is charged on balances over 90 days.
Please provide at least 2 business days notice for any appointment changes. Missed appointment fee of $50 per half hour applies to Club Members.

After this initial registration, we will ask you for formal updates every 2 years.

We do accept most PPO dental plans and are considered in network for Delta Dental. If you don’t currently have insurance coverage, we offer options:
*If you are 62 years or older, we offer a 10 percent courtesy off all our treatments.
